CONTACT US

Making positive changes and breaking negative habits is challenging and you don't have to do it alone.  You can call or stop by our office to schedule your first appointment.

SCHEDULE YOUR FIRST APPOINTMENT

When scheduling your first appointment, we will collect some intake information similar to medical offices you have visited, and obtain a copy of your insurance information.

Our treatment team will review your intake information and determine the best counselor to pair you with to address your or your family's unique needs.

During your first session, your therapist will get to know you, the areas that you are struggling in, and develop a treatment plan. Your therapist will check in with you regularly throughout the process to assess the effectiveness of the therapy, and make adjustments as needed.

Below are the documents required at your first appointment.
